Why Does My Pain Not Make Sense?
Central sensitization is an amplification of neural signaling within the central nervous system that makes the body hyper-responsive to pain and other sensory stimuli.
It acts like a faulty smoke alarm, triggering at maximum volume in the absence of smoke or fire. Someone experiencing central sensitization often feels heightened discomfort, which can make innocuous sensations painful. This heightened sensitivity can also contribute to emotional effects such as increased anxiety, irritability, or feeling easily overwhelmed.
For example, skin sensitive to touch may react strongly to light clothing, a gentle breeze, or light pressure of a hand, even when there is no injury.
Common experiences include:
- Hyperesthesia: Heightened sensitivity to touch or other sensory stimuli, where normal sensations feel amplified or unpleasant.
- Dysesthesia: This is an unpleasant or painful abnormal sensation that is often distressing. Examples include burning, crawling, electric shocks, and painful tingling.
- Allodynia: Pain from a stimulus that is normally not painful (e.g., light touch, clothing, or a gentle breeze). It is frequently seen in chronic pain conditions and high-impact disorders like CRPS.
- Hyperalgesia: An exaggerated pain response, where a mildly painful stimulus feels far more intense than it should.
- Dysesthesia: This is an unpleasant or painful abnormal sensation that is often distressing. Examples include burning, crawling, electric shocks, and painful tingling.
- Paresthesia: These are abnormal sensations that occur without an obvious external stimulus. They often feel like tingling, pins & needles, buzzing, or a ticking sensation.

Central sensitization can create a bidirectional amplification between the body’s protective systems, leading to a self-reinforcing cycle: pain signals heighten emotional distress, which increases muscle guarding and tension, which in turn amplifies pain.
These protective systems start reinforcing one another, putting the body on high alert even after the original threat is gone.
- The endocrine system elevates stress hormones, causing fatigue and mood swings.
- The immune system heightens inflammation, leading to widespread aches.
- The motor system experiences muscle guarding, altered posture, or avoidance movements.

Challenges In Care
This complex, self-reinforcing cycle is one reason central sensitization is frequently misunderstood or under-recognized in conventional medical settings. Many physicians are highly skilled at identifying structural problems, but the nervous system changes involved can be subtle and may not appear on standard tests.
When central sensitization is not fully recognized, you may face additional challenges in your care, such as:
- Delayed or Inappropriate Treatment: When care primarily focuses on peripheral issues, while the primary driver is CNS amplification, you may undergo unnecessary procedures that do not address the root problem.
- Over-reliance on Medications: Opioids and other painkillers may be prescribed for short-term relief, but they can sometimes worsen central sensitization over time. This may lead to opioid-induced hyperalgesia and increased risk of dependence, tolerance, and other side effects.
- Symptom Dismissal or Invalidation: When standard tests come back normal, you may hear comments like “nothing is wrong” or “it’s all in your head.” This can leave you feeling dismissed, which may contribute to increased anxiety, depression, loss of trust in healthcare, and avoidance of further care.
- Worsening Pain Cycle: Without validation and proper care, the self-perpetuating cycle can intensify (pain → guarding → more pain). This often leads to fear-avoidance, deconditioning, poor sleep, and elevated stress.
- Missed Opportunities for Effective Care: Conditions like fibromyalgia, CRPS, and many other forms of chronic pain respond best when pain neuroscience education and targeted strategies are part of the care plan.

Neuroplastic Changes
These are some of the neuroplastic changes that might occur during central sensitization.

Cortical Smudging
Central sensitization can contribute to something called cortical smudging. Over time, the constant amplified pain signals cause the brain’s sensory map of the body to become blurred or altered. This change, known as maladaptive neuroplasticity, can make pain and sensory symptoms feel more widespread or harder to pinpoint.

Impaired Pain Inhibition
The body’s natural ability to modulate or dampen incoming nociceptive signals can become less effective. This happens through descending pathways from the brain and local mechanisms in the spinal cord. As a result, pain often feels more intense and harder to control.

Neuronal Hyperexcitability
Nerve cells can become more easily excited, causing nociceptive signals to build up and linger longer than expected, sometimes described as a “wind-up” effect.

Glial Cell Activation
Support cells in the nervous system can become overactive and release substances that further increase sensitivity.

Limbic System Changes
The emotional centers of the brain can become more closely linked with pain processing. This can heighten feelings of anxiety, fear, frustration, or being easily overwhelmed.

Autonomic Nervous System Dysregulation
The body’s stress-response system can remain on high alert, leading to symptoms such as increased muscle tension, fatigue, racing heart, or digestive upset.

Sleep, Cognitive & Widespread Pain Changes
Ongoing pain activity can disrupt sleep, contribute to difficulty concentrating (“fibro fog”), and allow discomfort to spread more widely throughout the body.
