Nociceptive Signals Are Not Pain

Pain is an output, not an input. When your foot hurts, it is because your brain has decided there is potential danger and protection is needed. Like a protective parent, your brain will do whatever it believes is necessary to keep you safe. When it detects a threat, it uses pain and other protective responses as warning signals to prompt you to change your behavior.

Is All Pain Real?

Is all pain real? Yes. Even though pain is produced by the brain, it is completely real. The sensation, the distress, and the impact on your life are genuine. Understanding that pain is an output does not make the experience less valid. It simply helps explain why the intensity of pain does not always match the severity of your injury.

It is also common for people with persistent pain to say “My doctor says nothing is wrong but I still have pain,” or that the “MRI is normal but I’m still in pain.” This does not mean the pain is imaginary or just in your head. It often means your nervous system is still in a protective state, even when exams and imaging do not show a clear injury.

Pain Without Injury?

Many people wonder why pain continues after an injury heals. Sometimes your body sends nociceptive signals, yet you do not experience pain. At other times, pain continues long after you have healed. This happens because your brain relies on neurotags; these are networks of thoughts, memories, emotions, and sensory information to decide whether protection is needed.

If your brain links a current experience with a previously threatening pattern, it may generate pain even when the original danger is no longer present. This is one reason an old injury site can remain sensitive. Your brain is still treating the area as vulnerable and using discomfort as a protective strategy.

So the brain can create pain without an injury? Yes. The brain can produce pain in response to perceived threat, past experiences, stress, poor sleep, or emotional distress, even in the absence of physical harm. In these cases, the pain is still real, but the primary driver is the nervous system’s protective stance rather than injury.

Can the Brain Unlearn Pain Patterns?

Yes, because the brain learns through experience, it can also update its protective patterns. When it repeatedly receives evidence of safety through movement, touch, calm states, and new associations, it can become less reactive over time. This process is gradual and benefits from consistency.
